Still a pain in the neck after all this time: of mumps in a previously immunized adult diagnosed in the ED

      Mumps is a childhood disease described by Hippocrates more than 2000 years ago (Quinlisk. J Infect Dis. 2010;202:655–656). It is characterized by a febrile illness associated with painful and inflamed parotid glands. The causal agent is an RNA virus of the paramyxoviridae family. Mumps has reemerged as an infection that is affecting young people and adults in the developed world. Current national and global trends show that smaller outbreaks continue to occur (Quinlisk. J Infect Dis. 2010;202:655–656; Vygen et al. Euro Surveill. 2016;21:pii=30156). Recently, trends of outbreaks have occurred mostly in previously vaccinated individuals (http://www.cdc.gov/mmwr/preview/mmwrhtml/mm5905a1.htm). Several hypotheses have been suggested as to the reason including the waning immunity from the vaccine. Diagnosing mumps is difficult if based mainly on clinical acumen. We present a case of mumps in an immunized adult diagnosed in the emergency department, highlighting the importance of maintaining consideration and vigilance for this entity even in an immunized adult.
